Our Membership in the East Markham Family Health Organization (FHO)

Dr. Ye has joined a group of 15 local physicians (East Markham Family Health Organization) at 7 convenient locations, offering our enrolled patients significant advantages in accessing care. As part of our ongoing commitment to providing you with the highest level of care, we have instituted a number of changes at our office.

1. Telephone Advisory Service - accessed via Telehealth Ontario

• Medical information is available from the FHO doctor-on-call or a nurse
• Available evenings after 8pm and all day and night on weekends and holidays

2. After-Hours Clinics - for urgent medical problems that cannot be addressed during regular office hours:

• Every evening from 5-8pm Monday to Thursday
• 3 hours on weekends, usually on Saturday morning & usually from 9am – 12pm

Within this group we are able to communicate information about our encounters with each other’s patients, sending along a note regarding the nature of the concern, the findings, as well as any actions taken such as prescriptions, blood work or other tests.  This communication enables us to keep abreast of the health needs and concerns of our patients, providing continuity of care, which is an essential component of quality care.

An Important Note:

For this reason, we ask that you call our office first instead of seeking help outside of this group or at a walk-in clinic.  During our regular business hours on Monday through Friday, if you have an urgent health concern, please call our office first thing in the morning after 9:00am and we will do our best to give you a same day appointment.

How to Enroll with our East Markham Family Health Organization (FHO)

Complete our FHO form and register with one of our doctors as your family doctor.

For those who have already registered with us in the past will be automatically registered with our FHO, and don’t need to register again.

You cannot be signed up with more than one doctor at a time. So please do not sign any similar form if for some reason you choose to attend a walk-in clinic or see another doctor.  That would remove you from our FHO and your doctor would no longer be registered as your family doctor.

Please remember to bring your health card with you at every appointment and keep us up-to-date with your address and telephone information.

Please keep your pharmacy phone number on hand when you are calling the doctor-on-call after-hours, in case a prescription is needed.
